Christus Spohn offers CarePartners management program in Kingsville

CHRISTUS Spohn is now offering a community-based management program in Kingsville called CarePartners.
CarePartners is a program that offers assistance with the coordination of basic health care services and healthcare access for adults 19 and over who meet the following guidelines: no health insurance, Medicare, or Medicaid, limited household income, frequent ED visits, inpatient admission, 30-day readmissions to your hospital, chronic illness, or have trouble paying for health care.
As a member of the CarePartners Program, patients will be able to get help to navigate the healthcare system and receive the services you might need.
Patients will have a Community Health Worker whose job is to put you in touch with services that might be helpful, provide you with health education to learn to live a healthy life, and support you in the management of your health.
Community health workers do not take the place of your doctor, but may work with them to keep you as healthy as possible.
